Add questions
Select Add Question.
Enter a Name for the question.
Type the question in the text box.
Select the question Type.
Select Add Answer and type the answer.
Repeat step 5 to add more answers.
Select Save Changes.
In the Information tab, insert the question into the Description text box. This is required for the question to appear on the page.
